Both the old medical forms and the new medical forms will be acceptable for summer camp but we recommend that you use the new medical forms. If you use the old ones for summer camp, you will have to complete a new one by January 1, 2010 no matter what date you turned in the old one. Completing the old medical form will only cover your scout for activities through the end of this year. Completing the new medical form will cover your scout for one year from the date of completion. The new form can be found here. All scouts and adults will need to have Parts A, B and C completed for summer camp. Note, this is a PDF form.. You can type directly into this form and save the form, with the information you entered, onto your personal computer. If you are using the old medical forms, scouts will need to have Class 1 and 2 medical forms completed and all adults will have to have a Class 3 medical form completed. Troop 1011 Expense Reimbursement Form: Right click on this link and then select "Save Target As...". Save this Excel file to your computer, fill it in and print it. Bring the printed form with receipts attached to an Adult Troop Leader for processing. Skills Review Worksheet: Right click on this link and select "Save Target As...". Save this Excel file to your computer. This spread sheet provides a detail listing of most skills you need to qualify to advance to the next Boy Scout rank.
